Forms

Build pre-chat forms, ticket submission forms, and custom forms with drag-and-drop fields.

What's included

Replify ships with a form builder that lets you collect information before a chat starts, when a ticket is submitted, or on any custom surface. Forms are unlimited on both Free and Pro.

Default forms

Two forms are created automatically on activation:

  • Pre-chat form. Shown to visitors in the widget before they can start chatting. Collects name and email by default.
  • Ticket submission form. Shown when a visitor creates a ticket from the widget. Collects subject, description, and priority by default.

Both can be edited freely — add fields, remove fields, reorder them.

Field types

  • Text (single line)
  • Email
  • Phone
  • Select (dropdown)
  • Textarea (multi-line)
  • File upload (with type and size restrictions)
  • Checkbox
  • Radio buttons

Field options

  • Required toggle. Mark any field as required or optional.
  • Drag-and-drop ordering. Reorder fields visually.
  • Conditional visibility. Show or hide a field based on the value of another field.

Submissions viewer

Every form submission is stored and accessible in Replify → Forms. You can view the submitted data per response, filter by form, and see which chat or ticket resulted from the submission.

Custom forms

Beyond the two defaults, create as many forms as you need. Assign them to different widget surfaces or use them for internal data collection workflows.
